Osteoarthritis / संधिगतवात 

Osteoarthritis is a type of joint disease that results from breakdown of joint cartilage and underlying bone.

Formation of hard knobs at the middle finger joint known as -Bouchard's nods and at the farthest away finger joint known as Herberden's nodes. Both are the common feature of osteoarthritis in hand.

Cause-

Primary Osteoarthritis

• Age >40
• Previous joint injury
• Inherited factor
• Obesity
• Metabolic disorder 

Secondary Osteoarthritis

• Trauma
• Inflammatory disease
• Rickets in infants 
• Osteomalacia in adult

Pathophysiology-

Cartilage damage (no blood supply and not formed again) > bone rub with eathother (new bone out growth appears, osteophytes or bone spur) > Stag - osteoporosis > Bone damage > Osteoarthritis.

Sign and Symptom-

• Joint pain and stiffness 
• Joint swelling
• Decreasing rang of motion
• Pain intermittet over time (Good day and bad day)
• Related to movement and weight bearing joint
• Crepitus (craking sound)
• Brief, <15 minutes morning stiffness
• One or few joints are painful
• Finger nodes (nodular swelling)

Clinical sign -

Muscle weakness wasting

Restricted movement

No or only mild synovitis

Osteophytes around joint margin

Risk factor -

Overweight

Legs of a different length

Job with high level of joint stress

Investigation -

• X-Ray finding 
• ASLO Titre
• C- Reactive protien
• Bone mineral density test ( Indicate quantitative measurement of Ca and Mineral present in bone)
• Serum Calcium - Normal 8.5 to 10.2 mg/dl
• Serum vitamin D3 - 20 to 100 ng/ml

Complication-

• Easy to rupture
• Chronic back pain
• Difficulty in movement

Prevention - 

Opposite of cause

Prevent to fall

संधिगतवात

 

परिचय -

यह मुख्यतः वृद्ध या मध्य अवस्था में होता है। क्योकि इन अवस्था में मनुष्य शरीर में स्वाभाव से ही वात दोष की वृद्धि रहती है। 

अस्थि तथा वात का आश्रयाश्रयी सम्भन्ध - वात  दोष की विगुणता - अस्थियों में विकृति - सन्धिवात नामक रोग उत्पति। 

इसमें विशेषतः बड़ी सन्धिया (weight bearing joints ) अधिक प्रभावित होते है। 

यथा - जनु सन्धि (Knee joint )

         गुल्म सन्धि (Ankle joint )

परिभाषा -

वात दोष की विकृति के कारण सन्धि  में शूल होना, आकुंचन एवं  प्रसारण में कठिनाई तथा शोथ , संधिगतवात कहलाता है। 

निदान -

वात  दोष प्रकुपित होता  है ,

 वात  के सामान्य व विशेष निदान। 

सम्प्राप्ति -

वात प्रकोप निदान सेवन > वात दोष का प्रकोप > वात दोष का संधिस्थान में आश्रय > सन्धि (Joints ), स्नायु (ligament ), काण्डरा (Tendons ) की विकृति > संधिगतवात। 

सामान्य लक्षण - 

चलते समय सन्धियों में आवाज होना। 

साध्यासाध्यता -

रोगी  अवस्था में हो तो चिकित्सा - कृचछीसाध्य

अगर रोगी प्रारंभिक अवस्था में न हो तो चिकित्सा - याप्य / असाध्य।
